The current website has no site-operated account, intake form, advertising pixel, or analytics script. Interactive worksheets save entries in local storage in the user's browser; entries are not sent to the website. Users can export, print, or clear them.
Hosting and external services
The hosting or security provider may process ordinary technical logs such as IP address, requested URL, device information, and time of access. External agency links are governed by the destination's privacy policy.
Messages sent to [email protected] are received by email. Do not email Social Security numbers, passwords, complete medical records, unredacted personnel files, or confidential employer or customer data.
Control of worksheet data
Browser-saved worksheet data can remain on a shared device. Use the Clear saved copy control after exporting what you need. Clearing site data in the browser can also remove it.
